Wayne Memorial Hospital Patient Portal User Agreement

Patient Portal (“Patient Portal” or “Portal”) offers secure viewing and communication as a service to patients who wish to view portions of their records. Secure messaging can be a valuable communications tool, but has certain risks. By signing the Agreement to Abide by the Terms of Use, you accept the risks and agree to follow Terms of Use, as described below.

myWayneHealth Terms & Conditions of Use

The Wayne Memorial Hospital myWayneHealth service (“myWayneHealth”) is provided solely for the convenience of patients of Wayne Memorial Hospital. Your access to and use of myWayneHealth is subject to and governed by these terms and conditions (“Terms and Conditions” or “Agreement”) and applicable law, rules and regulations. Throughout this Agreement, Wayne Memorial Hospital will be referred to as “Wayne,” “we,” or “us.” Throughout this Agreement, users of myWayneHealth will be referred to as “you” or “users”.

NEVER USE MYWAYNEHEALTH FOR EMERGENCY OR OTHER URGENT MEDICAL ISSUES. IF YOU ARE EXPERIENCING A MEDICAL EMERGENCY OR NEED URGENT MEDICAL CARE, IMMEDIATELY CALL 911 (IF YOU ARE IN THE UNITED STATES), GO TO YOUR LOCAL EMERGENCY ROOM, AND/OR CALL YOUR PHYSICIAN.

ALL HEALTH AND HEALTH-RELATED INFORMATION IN MYWAYNEHEALTH IS INTENDED TO BE GENERAL IN NATURE, PROVIDED SOLELY FOR WAYNE PATIENT CONVENIENCE, AND IS NOT A SUBSTITUTE FOR A VISIT TO A LICENSED HEALTHCARE PROVIDER. PLEASE SEE YOUR PHYSICIAN, NOT MYWAYNEHEALTH, FOR ALL MEDICAL ADVICE AND CARE.

General Terms & Conditions

myWayneHealth currently provides a convenient way for you to do the following, subject to these Terms and Conditions and applicable law, rules and regulations:

– Have a web-based, read-only access to portions of your Wayne Memorial Hospital information.

Please note that myWayneHealth is not your complete medical record, nor is it your official Wayne Memorial Hospital medical record. By signing up for myWayneHealth, you agree to comply with this Agreement and applicable law, rules and regulations. Your access to and use of myWayneHealth is subject to your agreement to these Terms and Conditions and such law, rules and regulations.

The terms and conditions set forth in this Agreement may be viewed at any time by clicking the “Terms and Conditions” tab at the bottom of each page of the Site. In regard to myWayneHealth and this Site, where any provision(s) of this Agreement conflict with any other term, policy or condition published on or otherwise applicable to this Site or any other Wayne website, the relevant provision(s) of this Agreement shall control, prevail and be enforced.

We have the right to modify this Agreement at any time, in our sole discretion. In general, modifications to these Terms and Conditions will not apply retroactively and will become effective no sooner than fourteen days after they are posted. Your continued use of myWayneHealth constitutes your acceptance of all of the modifications we have made.

If you do not agree with our modifications to these Terms and Conditions or any of our changes to our Privacy Policy, you should discontinue your access and use of myWayneHealth. Neither the course of conduct between the parties nor trade practice shall act to modify any provision of this Agreement.
